As a part-time Hospice RN Case Manager, you will play a crucial role in enabling patients and their loved ones to live each day as fully and comfortably as possible.

Job Overview:
We are seeking a dedicated and compassionate RN Case Manager to join our team. As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care of assigned patients under the direction of the clinical director. This is an on-the-road position with mileage reimbursement, and you will be required to service the entire state of Rhode Island.
Responsibilities:
Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
Provide professional nursing care by utilizing all elements of the nursing process
Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
Supervise hospice aides and provide guidance on patient care
Collaborate with patients, families, and healthcare professionals to develop a comprehensive plan of care
Provide emotional and spiritual support to patients and families
Requirements:
Must possess an active and unencumbered RN license in Rhode Island
Must have a nursing degree from an accredited college, university, or school
Prior experience in hospice is preferred
