Independence Rehab & Nursing, a skilled nursing facility (SNF) in Philadelphia, PA, is seeking an experienced MDS Coordinator to join our team. The MDS Coordinator (MDS Nurse) oversees the Minimum Data Set (MDS) assessment process, working closely with our interdisciplinary team. Prior nursing home experience is required for the MDS Coordinator role.

MDS Coordinator Duties
Complete and coordinate MDS assessments accurately and on schedule
Manage assessment timelines under PDPM
Collaborate with the interdisciplinary team on resident care plans
Ensure documentation supports accurate reimbursement and compliance
Track and submit assessments per state and federal requirements

MDS Coordinator Qualifications
Registered Nurse (RN) with an active license
MDS experience required
Must have experience in a skilled nursing facility (SNF) / long-term care (LTC) setting
