This full-time position serves as a critical liaison between patients, their families, healthcare providers, and our caregiving staff. As a Patient Coordinator, you will be responsible for managing the complex logistics of home care delivery, ensuring that patients receive timely, appropriate, and culturally sensitive care. Your bilingual abilities will be essential in serving our diverse patient population and breaking down language barriers that often exist in healthcare settings.

Key ResponsibilitiesScheduling and Coordination

• Develop, maintain, and optimize comprehensive schedules for home care aides, nurses, and therapists, ensuring appropriate coverage for all assigned patients
• Coordinate patient appointments, medical visits, and therapy sessions while considering patient preferences, caregiver availability, and clinical requirements
• Manage last-minute schedule changes, call-outs, and emergency coverage needs with efficiency and professionalism
• Utilize scheduling software systems to track caregiver assignments, patient needs, and service hours
• Monitor and adjust schedules proactively to prevent gaps in care and ensure continuity of service
• Create and distribute weekly and monthly schedules to all relevant parties in a timely manner
• Maintain accurate records of all scheduling changes, cancellations, and modifications

Patient and Family Communication

• Serve as the primary point of contact for Russian-speaking patients and their families, providing information, support, and guidance in their preferred language
• Conduct intake assessments and gather comprehensive information about patient needs, preferences, and medical requirements
• Explain home care services, policies, and procedures to patients and families in both Russian and English
• Address patient concerns, complaints, and questions promptly and professionally, escalating issues when appropriate
