Family Nurse Practitioner

Job Overview:
HealthYes is seeking a compassionate and experienced Family Nurse Practitioner in the Great Neck area, NY. This position focuses on delivering patient-centered care, managing acute and chronic conditions, and promoting health and wellness within a collaborative healthcare environment.

Key Responsibilities:

• Conduct comprehensive patient assessments, including health histories and physical examinations.
• Diagnose and manage acute and chronic illnesses, developing personalized treatment plans.
• Address preventative care needs and close care gaps for assigned patients.
• Order and interpret diagnostic tests and lab results; manage follow-ups.
• Collaborate with healthcare professionals and community resources to ensure holistic care.
• Participate in quality improvement initiatives and departmental committees.
• Provide supervision and training to medical staff, including students and residents.

Qualifications:

• Master of Nursing from an accredited institution.
• Active New York State Family Nurse Practitioner license.
• Spanish or Mandarin speaker preferred.

Experience Requirements:

• 2-3 years of experience as a practicing Family Nurse Practitioner (FNP) preferred.
• Familiarity with medical devices (e.g., glucometers, blood pressure monitors).
• Experience with community resource facilitation and referrals is advantageous.
