Responsibilities:Coordinate and oversee the facility's Social Services department.
Complete resident assessments, care plans, and required documentation.
Assist with admissions, discharges, and care transitions.
Provide support, counseling, and resources to residents and their families.
Participate in care conferences and interdisciplinary team meetings.
Ensure compliance with state and federal regulations.
Advocate for resident rights and quality of life.
Build positive relationships with residents, families, hospitals, and community partners.
Long-term care or skilled nursing experience required
Knowledge of state and federal regulations related to long-term care.
Excellent communication, organization, and documentation skills.
Compassionate, resident-focused approach to care.
Ability to work independently while collaborating with an interdisciplinary team.
