About the Role:
The Hospital Liaison position serves as the primary point of contact between the skilled nursing facility and referring hospitals, case managers, discharge planners, and physicians. This role is responsible for building and maintaining strong referral relationships, conducting bedside patient/family assessments and education, and coordinating smooth, timely transitions of patients from hospitals into the SNF.
Responsibilities:
Cultivate and maintain strong working relationships with hospital case managers, discharge planners, social workers, and physicians to generate consistent patient referrals.
Serve as the face of the facility within assigned hospital(s), representing the SNF's services, capabilities, and culture professionally at all times.
Identify new referral opportunities and expand the facility's network of referral sources.
Requirements:
Degree in Healthcare Administration, Nursing, or related field preferred
Minimum 1-3 years of experience in a healthcare setting; preferably in hospital discharge planning, case management, SNF admissions, or healthcare sales/liaison work.
Working knowledge of Medicare, Medicaid, and managed care/insurance authorization processes for post-acute care.
