Position Summary:
The Case Manager – Discharge Coordinator coordinates patient case management issues with treatment team related to discharge planning. Helps patients to set up and implement discharge goals to facilitate ongoing recovery.

Required Knowledge, Skills, Licensure, Training & Travel Requirements (if applicable):
Education: Bachelor’s degree in mental health or other healthcare related field. Will be required to maintain certifications in CPR and CPI.
Work experience: Must have experience working in the mental health and substance abuse field preferred, but not required.
Knowledge, Skills & Abilities: Must possess good verbal and written communication skills. Should be computer literate. Completes assignments accurately, in a timely manner, and within procedural guidelines. Organizes and prioritizes work.
Interpersonal Skills: Possess excellent interpersonal skills to help facilitate the development of rapport with patients and their family members, referral sources; and to work cooperatively with in-house physicians, and administrative, clinical and support staff members. Displays self-direction by seeking solutions to problems. Seeks out and offers new ideas for improving processes. Deals constructively in stress.

Essential Job Duties

Performs direct and indirect patient case management duties to assist patients in maintaining recovery, personal dignity, and safety.
· Participate in discharge and aftercare planning for patients and aid patients in seeking therapeutic resources and placement.
· Plans and arrange admission to special care facilities as indicated by the individual patient’s needs, upon physician’s request.
· Coordinates and make necessary contacts with available community resources to ensure continuity of care for patients.
· Assists patients with post treatment needs as necessary in securing medications, personal items, arranging off-site medical appointments, transportation, post discharge housing needs, etc.
· Visits with potential post treatment referral sources to obtain information as to feasibility of agency to meet patient’s post discharge needs and communicates information with clinical staff.
· Coordinates aftercare treatment.
· Facilitate and document educational lecture groups in conjunction with other treatment team members.

Participate as an active member of the treatment team in the development, implementation, and evaluation of patient’s treatment plan from admission through discharge.
· Attend treatment team and staffing meetings to report on patient discharge planning.
· Collaborate with multi-disciplinary team to formulate and provide services which will facilitate patient progress towards goals outlined on a master treatment plan.
· Documents in patient medical record all services rendered on a daily basis.
· Actively participates in staff development.

Participate in Performance Improvement Process.

Serve as the Case Manager to assist patients with but not limited to:
· Communicates with family/payees.
· Facilitates accusation of housing needs following treatment.
· Compiles and keeps current data base of follow up discharge resources i.e. housing, vocational, educational, etc..
