Assist with psychosocial assessments for residents upon admission and throughout their stay.
Provide emotional support and counseling to residents and their families.
Participate in care plan meetings and collaborate with the interdisciplinary team.
Assist with discharge planning and coordinate community resources.
Help residents and families navigate healthcare services and available benefits.
Advocate for resident rights and promote resident satisfaction.
Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
Assist with resident adjustment, transition, and discharge needs.
Build positive relationships with residents, families, staff, and outside agencies.
Bachelor's degree in Social Work, Human Services, Psychology, or a related field.
Previous experience in a skilled nursing facility.
At Least 2 years of experience in Social Work.
Excellent communication, organizational, and interpersonal skills.
Compassionate, empathetic, and resident-focused.
Knowledge of long-term care regulations and discharge planning is a plus.
