This position plays a vital role in supporting residents and their families by addressing psychosocial needs, coordinating resources, and helping ensure a positive resident experience.

Responsibilities
  • Assist with psychosocial assessments for residents upon admission and throughout their stay.

  • Provide emotional support and counseling to residents and their families.

  • Participate in care plan meetings and collaborate with the interdisciplinary team.

  • Assist with discharge planning and coordinate community resources.

  • Help residents and families navigate healthcare services and available benefits.

  • Advocate for resident rights and promote resident satisfaction.

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.

  • Assist with resident adjustment, transition, and discharge needs.

  • Build positive relationships with residents, families, staff, and outside agencies.

Qualifications
  • Bachelor's degree in Social Work, Human Services, Psychology, or a related field.

  • Previous experience in a skilled nursing facility.

  • At Least 2 years of experience in Social Work.

  • Excellent communication, organizational, and interpersonal skills.

  • Compassionate, empathetic, and resident-focused.

  • Knowledge of long-term care regulations and discharge planning is a plus.
