Are you passionate about connecting people to the resources they need to thrive? The Delaware Division of Medicaid and Medical Assistance (DMMA) is looking for a compassionate, organized Health and Human Services Case Manager I to join its Newark team. In this entry-level case management role, you'll serve as a trusted advocate — helping individuals and families access the health, social, and community-based services that support their independence and well-being.
What You'll Do
- Interview clients and determine eligibility for health and human services and benefits.
- Conduct comprehensive assessments to identify clients' needs and develop personalized service plans.
- Coordinate with community partners and service providers to facilitate access to essential services.
- Maintain detailed case records and documentation to ensure efficient, effective service delivery.
- Advocate on behalf of clients to ensure their needs are met and their rights are protected.
- Provide emotional support and guidance, empowering clients to achieve their goals.
- Participate in team meetings and collaborate with colleagues to identify and implement process improvements.
What We're Looking For
- Associate's degree or higher in behavioral or social science, or a related field. (Candidates without a degree may also qualify with a combination of relevant experience — see full job requirements.)
- Experience in case management or direct service delivery preferred.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with diverse clients.
- Demonstrated problem-solving skills and a commitment to high-quality, client-centered care.
- Knowledge of community resources and social services programs.
- Proficiency in relevant software and data management tools.
- Ability to work effectively in stressful situations and conduct investigations as needed.
