Responsibilities:
Conduct thorough assessments to gather information for initial and ongoing care plans, ensuring accurate and timely data submission to federal and state agencies.
Develop and implement comprehensive care plans that address the unique needs and goals of each resident, collaborating with interdisciplinary team members to ensure seamless care transitions.
Maintain accurate and up-to-date records of resident care plans, assessments, and progress, adhering to regulatory guidelines and facility policies.
Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes.
Participate in Quality Improvement initiatives to identify areas for improvement and implement evidence-based practices that enhance resident care and satisfaction.
Assist with staff education and training to ensure knowledge and skills are up-to-date, fostering a culture of continuous learning and quality improvement.

Requirements:
2 years of experience in a healthcare facility, with a focus on post-acute care, rehabilitation, or skilled nursing care.

Strong communication and interpersonal skills, with the ability to effectively interact with residents, families, and healthcare professionals.
