Job Summary:

At Absolut Care of Gasport, we are seeking a compassionate and dedicated Registered Nurse (RN) Admissions Nurse to join our team and help provide exceptional care to our residents and their families. As an Admissions RN, you will be responsible for ensuring a seamless transition for new residents, coordinating the admission process, and providing individualized care plans and all required assessments. Responsibilities:

• Complete comprehensive nursing assessments for new admissions

• Coordinate and facilitate the admission process, ensuring a smooth transition for residents and their families

• Review medical records and clinical information to develop individualized plans of care

• Collaborate with physicians, residents, families, and the interdisciplinary team to deliver compassionate, resident-centered care

• Ensure accurate and timely documentation of all patient information

• Provide exceptional care and support to residents, prioritizing their physical, emotional, and social well-being

• Other duties as assigned by the DON or designee

Qualifications:

• Current RN license in New York State

• Strong clinical assessment and communication skills

• Excellent organizational and documentation skills

• Compassionate and resident-focused approach

• Ability to work effectively as part of a team

• Long-term care or admissions experience preferred
