At Richboro Rehabilitation & Nursing Center, we are seeking a compassionate and experienced Social Services Director to join our interdisciplinary team.

Richboro is dedicated to providing top-quality skilled nursing care for short-term rehabilitation and long-term care, and we are looking for a Social Services Director who shares our commitment to excellence.

Job Summary:

You will educate staff on the role of social services, lead the development of written care plans, and hold meetings with patients/residents, families, and significant others to provide support and information.

You will also perform discharge planning activities, establish relationships with community-based agencies, and maintain contact and referral flow.

Responsibilities:

• Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents

• Educate staff on the role of social services and participate in staff development activities

• Lead the development of written care plans for each resident's social service needs

• Hold meetings with patients/residents, families, and significant others to provide support and information

• Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers

• Establish relationships and maintain contact and referral flow with community-based agencies and services

Qualifications:

• Bachelor's degree from an accredited school of Social Work or related field

• Licensed in Social Work for the state of PA
