Schedule: Full-Time | Monday – Friday | 7:30 AM – 4:00 PM
Pay: $25.00 – $28.00 per hour

No two days are the same. One morning you may be conducting a client supervisory visit, the afternoon coaching a caregiver, documenting assessments, collaborating with our nursing team, or helping solve a scheduling challenge. As our Field Supervisor, you will serve as the primary support person for caregivers working in the field while ensuring our clients continue to receive exceptional care.
Responsibilities include:
Conduct supervisory visits with clients in their homes.

Provide occasional direct care to clients, ensuring client is receiving appropriate care and that the care plan reflects actual needs.
Build positive relationships with clients, families, and caregivers.
Shadow caregivers when appropriate to provide coaching and support.
Coach, mentor, and support caregivers in the field.
Early identification of potential care concerns.
Address performance concerns promptly through coaching and minor disciplinary follow-up.
Thorough and Professional documentation of client needs, supervisory visits, caregiver performance, and conversations.
Collaborate closely with our Director of Operations and nursing team.
Assist with caregiver scheduling and staffing needs when necessary.

Maintain accurate and timely documentation/records.

Previous supervisory or leadership experience.

Reliable personal vehicle.
Valid driver's license.
Current automobile insurance.
Ability to travel throughout the Portland metro area during the workday.
Ability to lift at least 40 pounds.
Proficiency with Microsoft Office and Teams
Strong typing skills.
Excellent verbal and written communication skills.
Ability to maintain confidentiality and professionalism.
Ability to pass all required background checks and employment screenings.

U.S. work authorization.
Previous caregiving experience.
Experience working in home care, healthcare, senior services, or other human services.
Experience working alongside nurses or other healthcare professionals.

Experience completing assessments, documentation, or case notes.
This is a full-time, in-person position.
This is not a remote or hybrid position.
Your time will be divided between:
Working in our Portland office
Traveling throughout the community to visit clients and caregivers
Reliable transportation is essential because schedules may change throughout the day based on client and caregiver needs.
