Position OverviewWorking alongside the Director of Social Services and interdisciplinary team, the Social Work Assistant supports residents and families through admissions, discharge planning, care conferences, and psychosocial support. This role is essential in maintaining resident dignity, emotional wellbeing, and smooth transitions of care.
CompensationMSW: $38/hour
BSW: $34/hour
Full-time and part-time opportunities available
Supportive leadership and team-oriented environment
Conduct psychosocial assessments
Assist with discharge planning and safe transitions
Participate in care plan meetings
Support residents and families with emotional and practical needs
Coordinate with nursing, admissions, and therapy departments
Ensure compliance with NYS and CMS regulations
BSW or MSW required
Nursing home or long-term care experience required
Strong communication and documentation skills
Compassionate, organized, and team-focused
Knowledge of discharge planning and community resources
