Areas of Responsibilities:
Resident Support and Oversight:
- Assist with personal care, nutrition, hygiene, and meal planning.
- Ensure safety and educate residents on their medical conditions and medication.
- Administer medication (including insulin/tube feeding as required by the residents’ treatment plans) according to New York State AMAP regulations.
- Support residents in scheduling medical appointments, advocating during visits. and ensure medical follow-ups are documented and communicated to the RN and residence manager.
- Provide first aid and emergency medical treatment when needed.
- Track and report medical/medication updates, behavior supports, daily goal implementation, and other required data and information.
- Complete monthly medical and medication audits with RN.
- Attend monthly staff meetings and bi-weekly IDT meetings.
- Participate in residents’ annual Life Plan meetings.
- Role model positive behavior and healthy interactions.
Incident Management:
- Participate in training related to incident prevention and documentation.
- Follow regulatory and agency policies for incident management and reporting.
Record Keeping:
- Document all services, medical appointments, and medications accurately in the electronic health record system.
- Maintain the medial records binders for your caseload.
- Maintain the resident “To-Go” book for medical appointments and emergency/urgent care visits.
- Complete daily service delivery notes and other required documentation.
Communication:
- Communicate effectively and professionally with residents, families, coworkers, and external teams such as day habilitation programs and care coordinators regarding medical needs, appointments, and changes in condition.
- Notify families of significant changes, including urgent care and hospital visits.
Miscellaneous:
- Complete all required annual and recommended trainings and maintain certifications (AMAP, CPR, etc.).
- Accept assignments from Residence Manager or designee.
Qualifications:
- High school diploma or GED + 2+ years of experience working with individuals with intellectual and developmental disabilities.
- AMAP certified.
- Knowledge of OPWDD medical and medication regulations.
- Strong written and verbal communication skills.
- Familiarity with electronic health records.
- NYS Driver’s License and ability to pass background checks.
Physical Demands:
- Ability to lift up to 50 pounds.
- Must be able to stand, bend, and lift throughout the day.
Schedule:
Monday-Friday 8am-4pm 40Hrs a week
East Meadow 8am-4pm and Massapequa 8am-4pm (The schedule wil be shared between both locations)
