Position Summary

The Social Worker is responsible for addressing the psychosocial needs of residents by providing emotional support, coordinating services, advocating for resident rights, and collaborating with the interdisciplinary care team to ensure the highest quality of care.

This role serves as an important resource for residents and their families while helping create a positive and supportive long-term care experience.

Responsibilities
  • Complete required psychosocial assessments, MDS documentation, care plans, progress notes, and other required documentation in accordance with federal, state, and facility regulations.

  • Assess residents' emotional, social, and psychological needs and develop appropriate interventions.

  • Participate in interdisciplinary care plan meetings and contribute to individualized resident care plans.

  • Provide counseling, emotional support, and guidance to residents and their families.

  • Assist residents with adjustment to long-term care placement and changes in health status.

  • Coordinate discharge planning and facilitate safe transitions of care.

  • Connect residents and families with community resources and support services.

  • Advocate for residents' rights and promote dignity, independence, and quality of life.

  • Maintain ongoing communication with residents, families, legal representatives, and the healthcare team.

  • Coordinate family meetings and encourage family involvement in the care planning process.

  • Collaborate with nursing, therapy, activities, physicians, and mental health providers to address residents' psychosocial needs.

  • Assist residents without family support by coordinating available community and facility resources.

  • Educate residents and families about available services, resident rights, and long-term care resources.

  • Maintain accurate, timely, and confidential documentation.

  • Build positive working relationships with residents, families, staff, physicians, and community agencies.

QualificationsRequired
  • Bachelor's degree in Social Work or a related human services field (Social Work preferred).

  • Minimum of one (1) year of experience in a healthcare or long-term care setting preferred.

  • Knowledge of federal and state regulations related to long-term care.

  • Excellent communication, organizational, and interpersonal skills.

  • Strong documentation and case management abilities.

  • Ability to maintain confidentiality and exercise sound professional judgment.

  • Compassionate, resident-focused approach to care.

  • Ability to work effectively as part of an interdisciplinary team.

Preferred
  • Experience working in a skilled nursing or rehabilitation facility.

  • Experience with MDS assessments and care planning.

  • Licensed Social Worker (LSW/LCSW) or eligibility for licensure, if applicable.
