Assist individuals with activities of daily living (ADLs), including personal hygiene, bathing, dressing, grooming, toileting, and meal preparation.
Support individuals in developing and maintaining independent living skills.
Promote individual choice, self-determination, and person-centered outcomes.

Provide companionship and emotional support.
Assist individuals with participating in community activities, social events, recreational opportunities, and employment-related activities.
Provide transportation to appointments, work, community outings, and other scheduled activities as needed.

Encourage the development of natural supports and community relationships.
Administer medications and complete medication documentation as trained and authorized.
Monitor and report changes in health, behavior, or overall well-being.
Implement Individual Service Plans (ISPs), Behavior Support Plans, and other support strategies.
Ensure a safe and clean environment while adhering to agency policies and DODD regulations.

Respond appropriately to emergencies and critical incidents.
Complete accurate and timely documentation, including service notes, incident reports, medication records, and daily logs.
Maintain confidentiality and protect individual rights in accordance with HIPAA and agency policies.
Participate in required trainings and maintain all certifications.
Attend team meetings and communicate effectively with supervisors, families, guardians, and other service providers.
High School Diploma or GED required.
Previous experience working with individuals with developmental disabilities preferred but not required.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Compassion, patience, and commitment to supporting individuals with disabilities.
Ability to follow written and verbal instructions.

Basic computer and documentation skills.
Must be at least 18 years of age.
Valid driver's license and acceptable driving record.
Ability to pass background checks required by DODD and agency policies.
Must maintain or willing to obtain required certifications and training, including CPR/First Aid and Ohio-specific training requirements.
Ability to lift, transfer, and assist individuals as required by the position.

May require weekends, holidays, and flexible scheduling based on individual needs.
Frequent standing, walking, bending, lifting, and assisting individuals with mobility needs.
Ability to safely perform transfers and other physical support activities.
Ability to accompany individuals in various community settings and activities.
