120-Bed Skilled Nursing Facility | Full-Time

Are you a compassionate, organized, and resident-focused social services professional looking to make a meaningful impact? Our 120-bed skilled nursing facility is seeking an experienced Social Services Director to lead our Social Services Department and advocate for the psychosocial well-being of our residents.

Position Summary

The Social Services Director is responsible for planning, organizing, and directing the Social Services Department in accordance with federal, state, and local regulations, as well as facility policies and procedures. This role ensures residents receive high-quality psychosocial support while promoting dignity, independence, and quality of life.

Key Responsibilities
  • Complete psychosocial assessments and develop individualized care plans.

  • Coordinate admissions, discharge planning, and safe transitions of care.

  • Advocate for residents' rights and address resident and family concerns.

  • Participate in interdisciplinary care plan meetings and quality improvement initiatives.

  • Provide counseling, crisis intervention, and emotional support to residents and families.

  • Coordinate community resources and referrals.

  • Assist with advance directives and resident decision-making as appropriate.

  • Ensure accurate, timely documentation and regulatory compliance.

  • Collaborate with nursing, therapy, physicians, case managers, and community partners to support positive resident outcomes.

  • Educate residents and families regarding available services and benefits.

Qualifications

  • Minimum of 2 years of experience in a skilled nursing facility, long-term care, rehabilitation, or healthcare setting preferred.

  • Knowledge of CMS regulations, discharge planning, care planning, and resident rights.

  • Strong communication, leadership, organizational, and problem-solving skills.

  • Compassionate, resident-centered approach with the ability to work collaboratively in a fast-paced environment.
