Facility: Chateau Rehabilitation and Nursing Center
Location: Rochelle Park, NJ
Schedule: Full-Time | Monday–Friday, 9:00 AM–5:00 PM
Salary: $90,000–$95,000 annually
Chateau Rehabilitation and Nursing Center is seeking an experienced, compassionate, and dedicated Social Services Director to lead our Social Services Department. In this leadership role, you will advocate for residents, support families, oversee discharge planning, and ensure the highest level of psychosocial care while maintaining compliance with all state and federal regulations.

What You'll DoDirect and oversee all social services programs within the facility
Lead discharge planning and coordinate community resources and post-discharge services
Assess residents' psychosocial needs and develop individualized care plans
Advocate for resident rights and ensure dignity, respect, and quality of life
Provide counseling, support, and education to residents and their families
Facilitate resident and family care conferences
Collaborate with physicians, nursing staff, therapy, and the interdisciplinary care team
Maintain compliance with all federal, state, and local long-term care regulations
Develop and maintain social services policies and procedures
Assist with surveys, audits, and regulatory compliance initiatives
Build and maintain partnerships with community agencies and healthcare organizations
Ensure timely and accurate documentation within the electronic medical record
Supervise and mentor Social Services staff as needed
Bachelor's degree required; Bachelor's or Master's Degree in Social Work (BSW/MSW) preferred
Minimum of 2 years of social work experience in a healthcare, rehabilitation, or long-term care setting
Prior supervisory or leadership experience preferred
Strong knowledge of skilled nursing facility regulations, discharge planning, and resident rights
Excellent communication, leadership, and organizational skills
Ability to build strong relationships with residents, families, staff, and community partners
