Cholla Medical Group is expanding its Population Health Division and is seeking compassionate, motivated, and mission-driven Nurse Practitioners to provide in-home primary care services to AHCCCS members throughout Maricopa and Pinal Counties.

This is an exceptional opportunity for Family Nurse Practitioners (FNPs) and Pediatric Nurse Practitioners (PNPs) who are passionate about preventive care, population health, value-based medicine, and improving access to healthcare for underserved communities.

Our providers deliver care directly in patients' homes, focusing on Annual Wellness Visits, Well-Child Visits, preventive screenings, chronic disease management, quality gap closure, and care coordination. You will be supported by a dedicated field Medical Assistant and provided with a company vehicle to maximize efficiency and patient care.

What You'll Do

• Conduct in-home Annual Wellness Visits (AWVs) for adult patients.
• Perform Well-Child Visits (WCVs) and preventive assessments for pediatric patients.
• Complete health risk assessments and comprehensive physical examinations.
• Identify and close HEDIS, AHCCCS, and payer quality gaps.
• Manage chronic conditions and provide ongoing follow-up care.
• Coordinate care with specialists, hospitals, community resources, and caregivers.
• Promote preventive care and patient engagement.
• Document visits electronically using our mobile EHR platform.
• Collaborate with an assigned Medical Assistant to deliver exceptional patient care.

Qualifications

Required

• Active Arizona Nurse Practitioner license
• National Nurse Practitioner certification
• Current DEA registration
• Current BLS certification
• Valid Arizona driver's license
• Strong clinical assessment and decision-making skills
• Ability to work independently in a mobile healthcare environment

Preferred

• Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)
• Experience in primary care, family medicine, pediatrics, geriatrics, home-based care, or population health
• Experience with Medicaid, AHCCCS, Medicare Advantage, or value-based care programs
• Bilingual English/Spanish strongly preferred

Schedule

• Full-time position
• 5–6 days per week
• Up to 8-hour shifts depending on patient volume and geographic assignment
• Flexible field-based work environment
