Primary Care Provider – Geriatric & Mobile Medicine
Billet Health | Phoenix Metro, AZ
$150,000 Base Salary + Performance Bonus | Full-Time | Monday–Friday
Billet Health is seeking an experienced NP, PA, MD, or DO to join our growing Mobile Primary Care team serving older adults throughout the Phoenix metropolitan area.
This is an excellent opportunity for a provider who wants clinical autonomy, meaningful patient relationships, and the ability to care for patients where they live—without the traditional high-volume primary care office environment.
What You'll Do
Provide comprehensive primary care to patients in:
- Assisted Living Facilities
- Skilled Nursing Facilities
- Memory Care Communities
- Group Homes
- Retirement Communities
- Private Residences
Your responsibilities will include:
- Comprehensive evaluations and physical examinations
- Diagnosis and management of acute and chronic conditions
- Development and management of individualized treatment plans
- Medication management and prescribing
- Review of labs, imaging, and other diagnostics
- Chronic disease management
- Care coordination with patients, families, facilities, and healthcare teams
- Transitional and preventive care
- Minor procedures and wound care, when appropriate
- Timely and accurate clinical documentation

What We're Looking For
- Active Arizona license as an NP, PA, MD, or DO
- DEA license with prescribing authority
- Board certification where applicable
- Minimum 1–2 years of clinical experience
- Family Medicine, Internal Medicine, Geriatrics, SNF, post-acute, or home-based care experience preferred
- Wound care experience strongly preferred
- Strong clinical judgment
- Ability to practice independently
- Excellent communication and patient relationship skills
- Comfortable providing care in community and residential settings
- Willingness to travel throughout the assigned Phoenix Metro territory
