We are seeking a dedicated Regional Community Liaison to support multiple skilled nursing facilities throughout Southern Colorado. This role serves as the connection between hospitals, physicians, case managers, discharge planners, referral sources, patients, families, and our facilities to ensure smooth transitions of care while building strong community partnerships.
The ideal candidate has experience in healthcare marketing, admissions, community outreach, long-term care, or hospital liaison services and enjoys developing relationships that support quality patient care and facility growth.

Build and maintain relationships with hospitals, physicians, case managers, social workers, discharge planners, and referral sources
Educate patients, families, and healthcare professionals about skilled nursing and rehabilitation services
Coordinate with admissions and clinical teams to facilitate appropriate referrals and seamless transitions of care
Travel throughout the region to hospitals, healthcare providers, and supported facilities
Represent the organization at networking events, community meetings, and outreach opportunities
Collaborate with facility leadership to support admissions, occupancy, and census goals
Serve as a trusted resource for referral partners and community organizations
Help strengthen community presence through relationship-building and ongoing engagement
Previous experience in healthcare marketing, admissions, hospital liaison, community liaison, or long-term care preferred
Skilled nursing or healthcare experience strongly preferred
Strong communication and relationship-building skills
Self-motivated with the ability to work independently
Reliable transportation required
Ability to travel throughout Southern Colorado
