Pulmonary Program Coordinator

About the Role:

  • Coordinate daily pulmonary program activities, including patient assessments, treatment planning, and implementation of care plans

  • Collaborate with the interdisciplinary team to develop and implement effective treatment plans for patients with pulmonary conditions

  • Participate in patient care conferences to review patient progress and adjust care plans as needed

  • Monitor and report on patient outcomes, identifying opportunities for improvement and implementing changes as necessary

  • Develop and maintain relationships with healthcare providers, diagnostic centers, and other relevant entities to ensure continuity of care

Responsibilities:

  • Patient Care: Provide high-quality patient care, adhering to established standards and policies

  • Communication: Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes

  • Documentation: Accurately document patient information, treatment plans, and outcomes in a timely and efficient manner

Requirements:

  • Experience: 1-2 years in a health care facility
