Job OverviewRiverbrook Healthcare Center is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. We are hiring Part-Time and Full-time Speech-Language Pathologists to provide high-quality therapy services in our skilled nursing and subacute rehabilitation setting.As a Speech-Language Pathologist, you will evaluate, diagnose, and treat residents with speech, language, cognitive-communication, voice, fluency, and swallowing disorders. You will collaborate with an interdisciplinary team to develop individualized treatment plans that promote functional recovery, safety, and improved quality of life following illness, injury, surgery, or hospitalization.Essential Duties and ResponsibilitiesPerform comprehensive evaluations of speech, language, cognitive-communication, and swallowing functionDevelop and implement individualized treatment plans based on physician orders, clinical findings, and resident goalsProvide evidence-based interventions for speech, language, cognition, voice, fluency, and dysphagia disordersAssess swallowing function and recommend appropriate diet textures, strategies, and safety precautionsMonitor resident progress and modify treatment plans to maximize functional outcomesComplete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelinesCollaborate with physicians, nursing staff, physical therapists, occupational therapists, dietitians, and other interdisciplinary team members to coordinate resident careEducate residents, families, and caregivers on communication strategies, swallowing safety, cognitive rehabilitation techniques, and home exercise programsParticipate in interdisciplinary care conferences, family meetings, and discharge planningMaintain compliance with all professional, regulatory, and facility policies and standardsSupport a resident-centered rehabilitation environment focused on quality outcomes and clinical excellenceQualificationsRequiredMaster's degree in Speech-Language Pathology from an accredited programCurrent Georgia Speech-Language Pathologist license in good standingCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; Clinical Fellowship (CF) candidates may be consideredCurrent CPR certification or willingness to obtain upon hireStrong clinical assessment, communication, and documentation skillsAbility to work effectively within an interdisciplinary healthcare teamPreferredExperience in skilled nursing, subacute rehabilitation, long-term care, or inpatient rehabilitationExperience with dysphagia management and cognitive-communication disordersFamiliarity with Medicare documentation and reimbursement guidelinesExperience working with geriatric and medically complex populations
