What You'll Do:

• Evaluate and treat residents with speech, language, voice, cognitive, and swallowing disorders

• Develop individualized treatment plans focused on maximizing independence and quality of life

• Collaborate with therapy, nursing, and interdisciplinary teams to provide exceptional resident care

• Complete timely and accurate documentation using Net Health

• Utilize PointClickCare (PCC) as part of the interdisciplinary care process

• Educate residents, families, and caregivers on treatment strategies and recommendations

What We're Looking For:

• Current Pennsylvania Speech-Language Pathologist license

• Certificate of Clinical Competence (CCC-SLP) required

• Previous Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience strongly preferred

• Experience with Net Health and PointClickCare (PCC) preferred

• Excellent communication, clinical, and teamwork skills

• Compassionate, dependable, and resident-focused
