Type: PRN/Per Diem

Pay: $75/visit

Work under the direction of the Director of Clinical Services and Clinical Team Manager
Deliver assigned functions of care in a professional and patient-focused manner

What You Bring:
Master's degree in Social Work (MSW) or Licensed as a Clinical Social Worker (LCSW) from an accredited school
2-3 years of experience required, with strong community knowledge
1+ year of home care experience preferred
Ability to exercise initiative, sound judgement, and independent decision-making
Strong collaboration and communication skills
