Responsibilities
- Plan and conduct segments of outpatient treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serve as a co-leader in an interdisciplinary approach to patient groups, coordinating care with other professionals.
- Perform initial patient evaluation, develop individualized treatment plans, and implement therapeutic interventions to promote functional recovery.
- Monitor patient progress, adjust treatment plans as needed, and document outcomes in accordance with regulatory and facility policies.
- Educate patients and families on treatment goals, exercises, and home exercise programs to support ongoing recovery.
- Collaborate with physicians, nurses, and other rehabilitation staff to optimize patient outcomes and maintain continuity of care.

Requirements
- Active physical therapy license to practice in California
- Outpatient physical therapy experience preferred
- Strong communication and documentation skills
- Ability to work in a multidisciplinary team and maintain collaborative relationships
- Availability to work a 40-hour week on a 4x10-hour day schedule
- Eligibility to work in the United States
