Job Summary:
Brookside Healthcare and Rehabilitation Center, a 120-bed long-term care facility located in Roslyn, Pennsylvania, is seeking a compassionate and highly skilled Physical Therapist to join our team. As a Physical Therapist with Brookside, you will have the opportunity to work in a state-of-the-art facility with a robust short-term rehab to home and long-term care populations. Our rehabilitation department is in-house, and we are committed to providing excellent care to our residents.
Responsibilities:
Evaluate residents and develop appropriate care plans
Monitor residents' progress and adjust treatments accordingly
Provide direct resident treatment in accordance with an established plan of care, including individual, concurrent, and group therapy
Provide excellent training for residents and caregivers
Document services provided within NetHealth on a timely basis
Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations
Exhibit courtesy, compassion, and respect to residents, families, caregivers, visitors, physicians, administrators, and co-workers in each and every interaction
Participate in clinical program development and implementation
Contribute to the team meeting and decision-making process as defined by the program
Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality outcomes
Participate in quality improvement activities as needed
May perform additional assigned administrative duties
Educational Requirements:
Graduated from an accredited program approved by your professional association
Current licensure by the applicable state agency
Experience Required:
Clinical experience in a skilled nursing facility is preferred
