Responsibilities
- Plan and conduct segments of treatment programs to restore and improve physical, social, and mental function, aligning with department objectives.
- Serve as a co-leader in an interdisciplinary approach to patient groups.
- Complete evaluations, establish individualized treatment plans, and apply appropriate clinical techniques and tools.
- Deliver therapy interventions using established protocols and document care and discharge plans, ensuring communication to all appropriate parties.
- Maintain comprehensive and timely clinical documentation; supervise assigned assistants as needed.
- Contribute to program quality and participate in ongoing interdisciplinary collaboration.

Requirements
- Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016, or foreign equivalent.
- Physical Therapy licensure in the state of practice: Required
- CPR certification or Basic Life Support (BLS) certification from an approved vendor: Required
- Two years’ physical therapy experience: Preferred
- EPIC EMR experience: Must have
- Education/Experience: Outpatient PT setting experience is preferred
