We are looking for a compassionate and organized Case Manager to support formerly homeless adults in a Contract position based in San Francisco, California. This role focuses on helping tenants maintain stable housing, access community-based services, and improve overall well-being through consistent engagement and individualized support. The ideal candidate brings strong documentation skills, sound judgment in crisis situations, and the ability to coordinate effectively with property teams and external service providers.

Responsibilities:
• Oversee a high-volume caseload of residents, providing individualized case management that promotes housing stability and progress toward personal goals.
• Meet with tenants regularly through outreach, intake follow-ups, and ongoing check-ins to evaluate needs and adjust support plans as circumstances change.
• Help residents secure and maintain public benefits, manage recurring rent obligations, and address barriers that may affect successful tenancy.
• Coordinate referrals to healthcare, behavioral health, employment, and community support programs, and encourage follow-through with recommended services.
• Partner with property staff during unit reviews and pest control visits to address habitability concerns and support tenant compliance.
• Organize tenant meetings, community activities, and engagement opportunities that strengthen resident connection and foster a supportive living environment.
• Respond appropriately to urgent tenant situations by using de-escalation techniques, documenting incidents, and escalating concerns when needed.
• Maintain accurate case records, service notes, reports, and client files while safeguarding confidentiality and meeting documentation standards.
• Work closely with internal teams and outside agencies to advocate for tenants and assist with organization-wide recertification or check distribution activities as assigned.• Bachelor’s degree required, or at least 2 years of verified case management experience serving homeless or formerly homeless individuals in place of a degree.
• Minimum of 1 year of experience supporting diverse, low-income, homeless, or formerly homeless adult populations.
• Prior background in case management, tenant support, outreach, or supportive housing services.
• Familiarity with San Francisco, California community resources and the ability to connect clients with appropriate local programs.
• Ability to manage organized documentation and case files for a large resident caseload with strong attention to detail.
• Proficiency with Microsoft Office applications and confidence in preparing reports, correspondence, and service documentation.
• Strong problem-solving, communication, and crisis response skills, including the ability to remain effective in challenging situations.
