LVN Home Health

The LVN Home Health professional provides essential nursing care to patients in a home health setting, ensuring their comfort, safety, and well-being. This role involves administering medications, monitoring vital signs, managing wound care, educating patients and families, and maintaining accurate documentation while effectively communicating with the healthcare team.

Responsibilities

• Provide direct patient care and support in the home
• Manage and administer medications as prescribed
• Perform wound dressing and monitor wound healing progress
• Monitor and record vital signs regularly
• Educate patients and families about care plans and health management
• Document patient information and clinical data accurately
• Communicate effectively with patients, families, and healthcare team members

Preferred Qualifications

• At least 1 year experience in licensed vocational nursing
• Skilled in wound care and medication administration
• Proficient in monitoring vital signs and infection control practices
• Familiarity with electronic health records (EHR)
• Strong communication, empathy, and time management skills
