PRN LPN/RN, Flexible Wound Care & Monthly Cath Care

Position Overview

This PRN role provides routine monthly catheter changes, basic wound care, and scheduled clinic visits for medically stable patients. Care is predictable, low‐intensity, and focused on consistency, dignity, and strong communication with caregivers and facility staff.

Responsibilities

• Perform routine monthly catheter changes (clinic and home settings)
• Provide basic wound care following established protocols
• Document visits clearly and accurately in Modivera’s EMR
• Communicate with caregivers, facility staff, and Modivera’s clinical team
• Support medically stable TBI/SCI patients in our weekly NeuroAxis Rehab clinic
• Maintain compliance with state and payer requirements
• Deliver care with dignity, consistency, and professionalism

Qualifications

• Active Pennsylvania LPN or RN license
• Strong communication and documentation skills
• Comfort with catheter care (training available if needed)
• Basic wound care experience preferred but not required
• Reliable transportation for home and facility visits
• Compassionate, patient‐centered approach to care
