Responsibilities
  • Patient Care & Evaluation: Conduct comprehensive initial evaluations for patients in hospital or home health settings. Design and implement individualized treatment plans focusing on functional mobility, strength, pain management, and physical rehabilitation.
  • Interdisciplinary Collaboration: Collaborate closely with physicians, nurses, case managers, and other healthcare professionals to coordinate comprehensive care and effective discharge or follow-up planning.
  • Treatment and Education: Modify treatment plans dynamically based on patient progress and medical status. Educate patients, families, and caregivers on safe mobility techniques, precautions, and home exercise programs.
  • Documentation: Accurately and efficiently document patient evaluations, daily treatments, progress notes, and discharge summaries in compliance with organizational policies, regulatory requirements, and industry standards.
  • Supervision: Provide clinical supervision, delegation guidance, and mentorship to Physical Therapist Assistants (PTAs) and therapy technicians within the department.
Requirements:

Qualifications
  • Education: Bachelor’s, Master’s, or Doctorate degree (DPT) in Physical Therapy from an accredited program.
  • Licensure: Valid state licensure as a Physical Therapist, or license eligible. Basic Life Support (BLS) certification required.
  • Skills: Strong clinical assessment, differential diagnosis, and reasoning abilities. Excellent communication, prioritization, and interpersonal skills in a hospital environment. Proficient in using hospital-specific therapy equipment, assistive devices, and electronic health record (EHR) systems.
