Job Summary:
Green Palms Health & Rehab of St. Marys is seeking a compassionate and skilled Clinical Liaison to join our dedicated team of healthcare professionals. As a Clinical Liaison, you will play a pivotal role in facilitating the transfer of patients from our skilled nursing and rehabilitation center to post-acute care settings, ensuring a seamless transition and optimizing patient outcomes. Responsibilities:
Develop and maintain relationships with hospital discharge planners, case managers, and other healthcare professionals to facilitate the transfer of patients to our center
Conduct assessments and develop individualized care plans for patients transitioning to our care
Communicate effectively with patients, families, and healthcare providers to ensure a comprehensive understanding of patient needs and treatment plans
Collaborate with our interdisciplinary team to develop and implement personalized care plans, addressing physical, emotional, and social needs
Provide education and support to patients and their families, empowering them to take an active role in their care
Participate in quality improvement initiatives, identifying opportunities to enhance patient outcomes and improve processes
Requirements:
Minimum Years of Experience: 0 (new graduates or experienced candidates welcome)
Level of Education: (High School Diploma or equivalent required; Bachelor's degree in Nursing or related field preferred)
