Clinical Evaluator - Per Diem
Job Summary:
We are seeking a per diem Clinical Evaluator to join our Admissions department at Vernon Rehabilitation and Healthcare Center in Vernon, Connecticut. As a Clinical Evaluator, you will play a crucial role in assessing the nursing needs of our patients and ensuring the delivery of high-quality care. This is a multi-facility role, working remotely, with support needed mostly on weekends.
Responsibilities:
Assess the nursing needs of patients and develop individualized care plans
Conduct thorough evaluations of patients' medical histories, mental and physical status, and rehabilitation potential
Collaborate with healthcare teams to ensure comprehensive care and planning
Maintain accurate and detailed records of patient assessments, care plans, and progress
Provide education and support to patients, families, and caregivers
Requirements:
Registered Nurse or Licensed Practical Nurse licensed in the State of Connecticut with at least three (3) years' experience in an acute care setting or Long Term Care experience preferred
Minimum of two (2) years marketing, admission, and/or clinical evaluator experience
Interest in the nursing needs of the chronically and/or acutely ill, with knowledge of the affect on the elderly
Ability to read, write, and comprehend English; ability to follow oral and written instructions
Strong written, verbal, analytical, and interpersonal skills
Proficiency with MS Office suite (e.g., Outlook, Word, Excel)
Ability to work hours as scheduled based on the requirements of the position assignment
