Location: Hybrid

Program: Mobile Crisis Response, Community Stabization, and/or Mental Health Skill Building

Population: Adults (18 and older)

Job Overview:

We are seeking a dedicated In Home Counselor to join our team. As an In Home Counselor, you will provide therapeutic intervention, skill restoration, and psychoeducation to adult clients. This is a hybrid (remote and in client’s home) hourly, PRN position, where you make your own schedule with clients.

Duties:

• Provide therapeutic services based on the client’s treatment plan.
• Provide education to clients and their families regarding mental health conditions, treatment options, and preventive measures
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive client care
• Document client progress on weekly case notes and quarterly reports.
• Monitor client progress and make necessary adjustments to treatment plans
• Participate in weekly team meetings and treatment planning.

Requirements:

• Bachelor degree in Psychology, Social Work, Counseling, Sociology, and any other related field
