JOB TITLE: CASE MANAGER – ENHANCED SERVICE PROJECT (ESP) - Supportive Housing Program
JOB TYPE: &10; Administrative &10; Operations &12; Programs &12; Full-Time
The Case Manager for the Enhanced Service Project (ESP) Supportive Housing program is responsible for providing case management services for single adult and families to maintain long-term housing and connect them to supportive services that address their barriers. The Case Manager works within an-inter-disciplinary team creating individualized housing and service plans and a referral of sources. This position will be based at Dignity Housing office in Germantown, Philadelphia and will serve supportive housing residents in scattered site settings.
OVERVIEW OF POSITION:
· Conduct monthly home site visits to residents and complete case service plan and unit inspection.
· Assist resident with medical access, credit repair services, transportation services, government benefits, and utility support.
· Document all resident contacts, case update, and referrals in HMIS and client charts timely.
· Conducts phone outreach and follow-up to referred residents to complete intake interview and assessment.
· Prepare reports and audits requested by Supervisor.
· Conducts routine audits of client files to ensure quality improvement and compliance with state standards.
· Demonstrate empathy and flexibility in responding to participant and staff needs.
· Ensure that deadlines are met and that the work produced is of the highest quality.
· Serves on special committees and complete other duties as assigned and/or required by the Program Manager or designee.
SUPERVISORY FUNCTIONS: N/A
EXPERIENCE AND NECESSARY SKILLS:
· Familiar with behaviors, attitudes and issues facing homeless populations
· Knowledge of public health benefits and community resources, and organizations addressing the issues of the population.
· Good written and verbal communications skills; excellent presentation skills.
· Familiarity with signs and symptoms of drug and/or alcohol dependency
· Must be culturally competent and sensitive towards diverse ethnic groups, cultural groups, and populations
· Experience supervising staff and volunteers
QUALIFICATIONS:
· Bachelor’s degree in Social Work, Sociology, Anthropology, Psychology, Ethnic Studies, other Human Services related degree; required.
· Minimum of two (2) years of professional experience provide counseling and case management services to individuals and families experiencing homelessness or other related populations; required.
· Ability to work at all sites and have a flexible schedule to include some evenings; required
· Required to have a working vehicle to conduct home visits and current Pennsylvania Driving License.
· Demonstrate proficient computer skills and familiarity with Microsoft office software, HMIS application
