Office & Field LPN

We are seeking a Licensed Practical Nurse (LPN) with Home Health experience to provide both office-based nursing care and independent home visits within the local area. This role involves a balanced mix of office duties and hands-on patient care in their homes, supporting patient health through medication administration, vital signs monitoring, and patient education.

Responsibilities

• Conduct patient visits independently in the local area
• Provide nursing care and support within the office setting
• Administer medications and monitor vital signs during visits
• Maintain accurate and detailed patient records
• Educate patients on health management and care plans
• Communicate effectively with patients and healthcare team members
• Utilize standard medical supplies during home health visits

Required Qualifications

• Licensed Practical Nurse (LPN) with a valid LPN license
• Minimum of 1 year experience in licensed practical nursing
• Experience in Home Health Care
