Responsibilities
- Perform thorough patient assessments and clinical diagnoses
- Develop and implement treatment and medication plans
- Manage patient medication and monitor therapeutic outcomes
- Educate patients on health conditions and preventive care
- Coordinate care among multidisciplinary healthcare teams
- Maintain accurate and up-to-date electronic health records
- Promote health and wellness through patient counseling and education
- Provide timely follow-up care and reassessments

Preferred Qualifications
- Entry-level experience in practice
