Experienced LPN for Assisted Living Facility

Compass Healthcare Consulting & Placement is conducting a search for an experienced LPN for an Assisted Living Facility located in Staten Island, NY. Qualified candidates will have prior LPN experience within Assisted Living, Nursing Home, Acute Care or Home Health Care. Openings available for Shifts 11:30AM - 8:00PM Monday through Friday. Licensed Practical Nurse (LPN)

Responsibilities:

• Provide supervision and direction to Home Health Aides

• Deliver nursing care in accordance with agency policies and procedures.

• Participate in incident reporting and follow-up actions.

• Completes, maintains and submits accurate clinical records and prepares oral reports.

• Assist in preparing and evaluating nursing care plans.

• Observe, document, and report changes in residents’ conditions

• Perform nursing procedures, including medication administration and treatments.

• Manage medication for residents, including changes, new prescriptions, and refills.

• Assists Nursing management with the assessments and reassessments of residents to determine health needs.

• Fosters a positive working environment, promoting teamwork and staff development

• Addresses employee concerns and ensures that staff adhere to organizational policies.

• Ensures a safe and healthy living environment for all residents.

• Undertakes resident/family teaching for self-care and continuity of care.

Qualifications/Skills:

• Graduate of an accredited practical nursing program.

• Valid New York State License as a Licensed Practical Nurse (LPN).

• Minimum of 1-2 years of experience in community nursing, acute care, or adult care facilities.

• Supervisory experience and medication management preferred
