Social Worker (LBSW) / RN Care Manager - Full-Time Days
Job Description
We are hiring for Social Worker (LBSW) and RN Care Manager positions. Both roles are responsible for coordinating patient care, discharge planning, assessing patient needs, and collaborating with the healthcare team to ensure quality, safe, and timely care.
Qualifications
Social Worker (LBSW)
- Bachelor's Degree in Social Work or related field
- Active Kansas LBSW License
- Hospital-based social work experience preferred
RN Care Manager
- Associate Degree or Diploma in Nursing (BSN preferred)
- Active Kansas RN License
- Minimum 2 years of acute care Medical-Surgical nursing experience
