Schedule: M–F, 8 AM–5 PM; 40 hours not guaranteed
On-call: 1 week/month after hours
License: Virginia RN or compact license
Territory: Patrick, Henry, Pittsylvania & Franklin counties

Skills: Home health, HCHB, OASIS, wound care/VAC, IVs, lab draws
Duties: Assess high-risk patients, coordinate care plans/transitions, collaborate with providers, arrange support services, monitor outcomes, and advocate for patients
Education: ADN or BSN in Nursing/related field
Requirements: Unrestricted RN license, strong clinical judgment, case-management certification
