Responsibilities:
- Provide direct patient care including monitoring vital signs, administering medications, and documenting clinical findings
- Assist with pulmonary assessments, spirometry, and respiratory therapy procedures
- Collaborate with physicians and clinical staff to develop and implement individualized patient care plans
- Educate patients and families on diagnoses, treatment plans, and disease management strategies
- Maintain accurate and up-to-date electronic health records (EHR) in compliance with HIPAA regulations
- Triage patient calls and coordinate follow-up care and referrals
- Active LPN license in the state of Pennsylvania (required)
- Graduate of an accredited Licensed Practical Nursing program
- Previous clinical nursing experience, preferably in pulmonology, primary care, or specialty care
- Proficiency with electronic health record (EHR) systems
- Strong communication, critical thinking, and patient advocacy skills
- Current CPR/BLS certification
- Compassionate, detail-oriented, and able to thrive in a fast-paced clinical environment
