Physical Therapist PT

This position is for a skilled nursing facility in Houston, which has both short-term and long-term patients. Golfcrest Nursing and Rehabilitation is under new ownership and has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.

Physical Therapist Job Responsibilities:

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.

• Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.

• Assist as needed with patient screenings for the need for Physical Therapy.

• Communicate progress and any barriers toward PT goals with interdisciplinary team members.

• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.

• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.

• Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.

Schedule: Full-time

Education, Experience, and Licensing Requirements:

• Current Texas licensure as a PT is required
