Physical Therapist (PT) - SNF

Job Details:

• Title: Physical Therapist (PT)
• Employment Type: Permanent | Full - Time
• Location: Pleasant Prairie, WI 53158
• Schedule: 8:30 AM – 4:30 PM
• Salary Range: $80,000 -$95,000 annually (Based on experience) + Benefits

Setting:

• Skilled Nursing Facility (SNF)
• Post-Acute Rehabilitation

-> Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled therapy interventions to improve mobility, strength, balance, coordination, and functional independence.
• Treat patients with a variety of medical conditions, including orthopedic, neurological, and medically complex diagnoses.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation according to facility and regulatory requirements.
• Educate patients and caregivers on therapy goals, safety strategies, and home exercise programs.
• Participate in care planning and maintain compliance with Medicare and state guidelines.

-> Qualifications

• Active Wisconsin Physical Therapist (PT) license or eligibility to obtain one.
• Graduate from an accredited Physical Therapy program (DPT preferred).
• Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehab, or Post-Acute Care preferred.
• Strong clinical, communication, and documentation skills.
• CPR/BLS certification preferred.
