Hospice Admissions & Case Management RN

Summary

We are hiring a Hospice RN Case Manager for a full-time, permanent opportunity in Denton County, TX. This position is ideal for compassionate Registered Nurses with hospice experience who are dedicated to providing exceptional end-of-life care to patients in their homes while supporting families throughout the hospice journey.

Location

• Denton County, TX

Salary

$85,000 – $95,000 annually

Additional Compensation

• Mileage Reimbursement: $0.65 per mile
• Cell Phone Reimbursement: $60/month

Schedule

• Full-Time
• Monday–Friday
• On-call rotation required
• Travel throughout assigned territory

Key Responsibilities

• Provide skilled nursing care based on individualized care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Administer medications and treatments as prescribed.
• Educate patients and families on hospice philosophy and symptom management.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Maintain accurate and timely EMR documentation.
• Respond to urgent patient needs and provide crisis intervention.
• Ensure compliance with hospice regulations and quality standards.

Requirements

• Active Texas Registered Nurse (RN) License.
• Minimum 1 year of Hospice nursing experience.
• Minimum 1 year of Case Management experience.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication and patient education skills.
• Experience with EMR systems.
• WellSky (Consolo) experience preferred.
