Job Title: Case Manager/Primary Therapist

Qualifications:

· Must be Certified Alcohol and Drug Counselor (CADC)

· Previous experience in substance use disorder (SUD) treatment.

· Knowledge of person centered, strength-based counseling, case coordination, and treatment/recovery planning using ASAM Criteria.

· Excellent communication, writing, and organizational skills.

· Working knowledge of Electronic Health Records, Kipu preferred and Microsoft Office.

· Minimum of 12 months of continuous sobriety required for persons recovering from a substance use disorder.

· Submit to DHS Schedule L background checks and meet the eligibility requirements for employment.

· Bilingual preferred.

Responsibilities & Duties:

I. Provide person-centered support, treatment/recovery planning assistance, and care coordination for assigned caseload.

· Collaborate and assist individuals and their family/significant other(s) in developing and Implementing their Initial Treatment Plan and Continuing Recovery Plan.

· Be a resource to the individual in identifying the treatment methods (e.g., groups, assignments, etc.) and the other recovery support services needed to meet their recovery goals and objectives.

· Provide care coordination needed for the individual and family. Be an informational resource and act as advocate to obtain support services during treatment and following discharge from treatment.

· Provide crisis intervention services to the individual and family as needed.

· Collaborate with the individual and others involved in his/her recovery to evaluate treatment effectiveness and update the Treatment/Recovery Plans throughout the course of treatment and make changes as needed.

· Provide progress and other reports to those involved in the individual’s treatment (e.g., DCFS, DHS, TASC, courts, probation, employers, etc.)

· Coordinate care with outside agencies to provide other services as needed to the individual.

· Provide utilization review updates to insurance or other managed care providers.

II. Provide person-centered counseling and education services.

· Conduct individual counseling sessions.

· Facilitate group counseling sessions.

· Develop and present educational materials utilizing individual assignments and lectures on a variety of recovery-based topics including substance use disorders, recovery planning, relapse prevention, living skills, and other relevant topics.

· Provide activities to family members or significant others such as family conferences or organized “Family Nights.”

· Use non-confrontational motivational strategies to promote engagement and retention in treatment, adherence to rules, and positive change.

III. Complete all documentation in a timely manner

· Clinical records

· Incident reports

· Referrals for Nursing/Medical, Mental Health, or other services

· Program related logs

· Staff service reports

· Other documentation as required

IV. Meetings, In-Services, Conferences and Committees

· Attend and/or present in-services or workshops as scheduled

· Attend and/or present staff meetings as scheduled

· Participate in various organizational committees as requested.

V. Other Responsibilities

· Provide programmatic/treatment coverage and services and carry additional caseload when necessary (e.g., vacation, illnesses, personal emergencies, etc.)

· Provide transportation as necessary.

· Supervise clients in various activities.

· Perform urine and breathalyzer tests according to procedures.

· Comply with all applicable Client Accounts procedures.

· Other duties as assigned by supervisor.
