Qualifications:
- Education and/or work experience in palliative care preferred.
- Must have excellent skills in assessment, diagnosis, and treatment, with strong communication abilities.
- Self-directed, motivated, and highly organized.
- Must meet the educational requirements for RN and Certified Nurse Practitioner licensure in the state of California.
Responsibilities:
- Oversee and manage the day-to-day operations of the palliative care program.
- Coordinate and oversee all direct and indirect patient services provided by clinical staff.
- Develop, implement, and evaluate the orientation program for new staff members.
- Mentor clinical team members, helping to enhance their skills and techniques for exceptional patient care.
- Assist in the development and administration of organizational goals, policies, and procedures.
- Provide comprehensive palliative care, including health history assessments, diagnosing conditions, managing health issues, and collaborating with patients, families, and other healthcare providers to develop individualized care plans.
- Build partnerships with community resources to address the needs of individuals and families.
- Under physician supervision, conduct patient visits, perform physical and face-to-face assessments, and initiate the development of the palliative care plan.
- Manage acute and chronic health conditions, including terminal illnesses, by developing personalized care plans, prescribing medications (as permitted), obtaining consultations, and coordinating hospice care services.
