Do you want to have a lasting impact on their future?Hope For Youth is seeking a dedicated Therapist to join our Youth Assertive Community Treatment (ACT) Program. Youth ACT is a program licensed by the New York State Office of Mental Health and designed to address the significant needs of children ages 10 to 21 who are at risk of entering-or returning home from-high-intensity services such as inpatient or residential treatment. Using a multi-disciplinary team approach, Youth ACT provides intensive, community-based support to help children with serious psychiatric needs remain safely in their homes whenever possible. Key Responsibilities:Function within the policies and procedures as outlined in program practice guide and employee manualAdhere to state regulations and applicable code of conductWork within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomesConduct person-centered/individualized Family, Individual, and Group Therapy in the home, community and office spacesEnsure that service delivery is in a trauma informed, culturally and linguistically competent mannerAttend trainings and professional development as assignedLiaise with DSS, schools and other child and youth collateral contactsAttend daily team meetings and family meetings as assignedConduct comprehensive clinical assessments to diagnose and/or determine needsDevelop a treatment plan with the child/family to restore functioning and/or ameliorate behavioral health symptom.Apply agency-identified Evidence Based Interventions best suited to meet the client's needs.Provide transportation for client to their service location as necessary.Provide crisis management and be on a rotation to cover crisis outside normal business hours.Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations How You'll Spend Your Time:60% Direct client therapeutic contact including transportation20% Case documentation, reports, case related meetings20% Staff/team meetings/supervision/training To qualify for this job, you need to be or have a minimum:LMSW/LCSW/LMHC/LMFT, limited permits1 years' experience working with children/adolescents and families preferredComputer proficiency, including Microsoft Office Suite, and experience working with EHRs.Valid NYS driver's license in good standingPhysical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, and ongoing daily access to own vehicle This opportunity is being offered as Full Time, 35 hour work week (flexible hours) Only candidates selected for an interview will be contacted.
