Overview

A MAT Discharge Planner is responsible for providing individual and/or group treatment for identified MAT patients scheduled to be released into the community, as well as coordinating and implementing discharge planning for inmates. The role requires effective communication with facility staff and community resources, and accountability for the services provided. Transition planning, which includes identifying local resources and coordinating appointments with programs and/or providers, must begin at least three months prior to the patient's release from the system.


Responsibilities

  • Provide individual and group treatment for identified MAT patients scheduled to release into the community, while also responsible for discharge planning in accordance with policy and procedures.
  • Maintain relationships with community agencies for individuals with mental illness or serious medical conditions, and facilitate applications of eligibility for financial assistance.
  • Coordinate medication and medical needs upon release, including potential residential placement, and assist inmates in making appointments with mental health agencies.
  • Communicate and maintain positive working relationships with appropriate Unit Team Staff and Institutional Parole Officers, documenting all services provided to the inmate in the health record.
  • Attend meetings relevant to release planning as directed by the Mental Health Coordinator, Clinical Supervisor or Mental Health Director, and comply with employee standards of the Facility.

Qualifications

Education

  • Master’s degree in social work or closely related field from an accredited college or university.

Experience

  • Previous experience in developing successful transition plans involving community resources for persons with mental illness. Must also have experience in social work case management in a mental health agency for mentally ill population.

Licenses/Certifications

  • Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
  • Must be eligible for LMHC or LICSW licensure, and be eligible for CADC or LADC certification/licensure.
